Declaration by Parent / Guardian

1. I/We solemnly affirm that all the particulars stated in the registration form are true and correct. In case of any incorrect information, I/We will always hold myself/ourselves responsible and accept the terms implied.

2. I/We hereby also declare that I/We will always adhere to the rules & regulations of the school as amended time to time and I/We will accept the decision taken by the school administration as final.

3. I/We have filled all the entries in the form accurately. I/We understand that the incomplete form may be treated as invalid application form liable to be rejected by the school.

4. The Society / School reserves the right to accept / reject any registration form without assigning any reason.

5. Submission of registration form by parent and its receipt by school, is not confirmation of admission in school.

When the admission of child is declared final, self attested copies of the following documents to be submitted along with original ones for the verification.

• Original Birth Certificate (Issued by Nagar Nigam) - Compulsory

• Proof of Residence - Compulsory

• Proof of annual income of family. - Compulsory

• Two recent passport size photograph of the child - Compulsory

• Photocopy of Mark sheet of class last attended - Compulsory

• Blood Group Report - Compulsory

• Certificate of physically handicapped - If applicable

• Certificate of Minority - If applicable

• Certificate of caste - If applicable

Note:- Please note that a login ID and password will be sent to your email id. You can use the same for the purpose of updating partly filled form before making payment. Please be aware that after making payment, nothing can be changed in the information filled in form. You can download the form also.
